Mr. NKURANGA LEOPORD GAHIMA, 49 years old, came for consultation from me on the 16th February 2012 complaining of:
Varus deformity of the left thigh with as genu varum of the same side
Stiffness of the left knee with limited flexing at 45 degrees
Limping on the same left side
The Patient’s Past History:
Road traffic accident in September 2008:
He had a comminutive compound fracture of the left femur which was treated
with an external fixators.
He got an osteomyelitis of the left femur as a complication.
In 2010, he was operated upon: Sequestrectomy and curettage were done in Tanzania. The said infection was cured.
Today the clinical examination shows that the fracture site has been consolidated with an obvious deformity of the left thigh, with a stiff left knee, limited left knee flexion at 45 degrees, and a 5 cm shortening at the left lower of the same left limb.
The X-Ray examination films show a vicious consolidation of the fracture site, but a callous is not solid. It also shows a varus deformity with an axial deviation of 30 degrees.
Conclusion
The deformity of the left lower limb is important with the left knee stiffness and a shortening of the left lower limb that is very important too. The surgical interventions consisting of the osteotomy, the bone grafts, and stable fixation (Limb deformation fixation and lengthening) can be done and followed by physiotherapy in a well equipped orthopaedics surgical centre.
