Cardiology in CMC looks after adults and children with many forms of heart and blood vessel problems. Patients who are critically ill, very sick, will be admitted to the Coronary Care Unit. This makes it quick and easy for sick patients with heart problems to get medical care.

Cardiology look after:

Ischemic heart disease where the blood supply to the heart is poor. Patients get angina, shortness of breath, swollen ankles etc.
Heart failure – when the heart does not work properly. People may get tired, short of breath, swollen ankles etc
Valvular heart disease – the valves in the heart have an opening that is too small or the valves leak.
Hypertension – high blood pressure (Increased BP).
Cardiac rhythm abnormalities – the heart beats too fast or too slow or irregularly.
Congenital heart problems – when someone is born with heart problems.
Vascular problems – the blood vessels in the body can become narrow or blocked. This often causes pain. Cardiology can open some of these blocks and so make the blood supply better.
What are the tests performed in Cardiology?

Non-invasive investigations(tests that do not need needles or operations) include:

An electrocardiogram (ECG) records the electrical patterns of the heart.
Echocardiography (Echo), an ultrasound scan of the heart.
Stress testing, TMT.
Holter test. This records the ECG while the person is walking around for one day i.e., 24hrs.
Ambulatory blood pressure monitoring - records the BP while the person is walking around for one day.
Head Up Tilt Test (HUTT) – to evaluate causes of fainting
Invasive Cardiology

There are two modern cardiac catheterization laboratories. In the cardiac catheterisation laboratories, often just called the cath labs they do invasive tests and invasive procedures.

Invasive tests involve putting needles, wires or tubes into the body.

Cardiac catheterization - the study of the heart by putting a tiny tube into the heart. Then the doctor injects a “dye” that shows the inside of the heart.
Coronary angiogram – injecting a “dye” to show the arteries of the heart.
Electrophysiological procedures – measuring and treating abnormal electrical activity in the heart.
Invasive Procedures

Coronary angioplasty. This is a treatment where special contrast is put into the heart to show the arteries on an Xray. The doctor widens any narrow arteries. This enlarges narrow areas or removes blocks. The blood can then flow faster.
Peripheral vessel angioplasty – looking at the arteries far away from the heart (peripheral). Then opening up any narrowed or blocked arteries (angioplasty).
Balloon valvuloplasty – opening up narrow heart valves – mitral, aortic, pulmonary and tricuspid valves.
Alcohol septal ablation.
Defect closures – abnormal holes in the heart can be closed. This includes Atrial Septal Defect (ASD); Ventricular Septal Defect (VSD) and a Patent Ductus Arteriosus (PDA).
Radiofrequency ablation – using a special radio machine to remove abnormal areas, which produce abnormal rhythm of the heart.
Pacemaker implantation - putting in an electrical machine to help the heart beat at the right speed. Temporary ones are just for a short time and are then taken out. Permanent ones stay in the patient and the patient goes home with it.
Intra-aortic balloon pump – used to treat some heart problems.

Hi Preeti Sinha,

it is better to take your father to CMC itself
They would have known the past problems of your father
So they may treat correctly
It is always better to take opinion from one doctor.

The USG report of my father in law are as follows;
-• Very distended GB with grossly abnormal GB wall. The GB wall is thickened and irregular.
• There is a heterogeneous low attenuation lesion seen measuring about 4.3X4.23 cms seen in segment 5 of the liver. This lesion demonstrate avidly enhancing thick and irregular capsule. Lesion is adjacent to GB.
• Lymph nodes seen around the coeliac axis and at the hepatic hilum.
• D/D Neoplastic ( GB mass with direct infiltration of the liver); inflammatory ( emphyema of GB with abscess of the liver).
• Correlation with clinical finding and FNAC would be advisable.
• A few calculi seen within the GB.

The FNAC report is as follows:
Specimen: USG guided FNAC from GB mass

Aspiration smears shows clusters and fragments of the typical cells with moderate amount of cytoplasma and eccentric nuclei and prominent nucleoli. There is a moderate cellular pleomorphism. Necrosis is also seen.

IMPRESSION:
Compatible with adenocarcinoma.

Chemotherapy is started from 1st April 2011.

I want the proper treatment if possible at the Christian Medical Hospital.

Please send suggestions as soon as possible.

waiting for quick reply,
Rana Khadka

Dear Dr, My mother had buccal squamous carcinoma in 2008. Surgery was done at ramakrishna hospital to remove it and neck dissection also was done lymph nodes were removed in the right side. Dr advised no need for radiation. It has not been spread anywhere. In June 2011 she was not able to open her mouth completely then Dr said it has occurred again should do a surgery. We agreed surgery was done August 25th 2011. They removed the upper and lower jaw kept plate inside for her to open the mouth and did aFLAP surgery tissues taken from breast.
After she was discharged a week later in the left side of the neck (where vein was taken for drips during surgery) a small tumor started to appear it was painful so again we went to ramakrishna biopsy was done and the result was metastatic deposit squamous carcinoma.
My mother is not willing to go to ramakrishna again.
We would like to come to CMC for treatment now. Dr please advise me . We all are very depressed.

Dear Sir/Mdm,
My mother Nasima Akther, age 52 years, living in Bangladesh has been recently diagnosed with breast cancer. I present a concise representation of her disease history along with attachments of her medical reports for your kind consultation as follows:
Case History:

1. Complaint about small painful lump 5 cm in Dec 2011
7. FNAC in Dec 2011: Ductal carcinoma(as attached)
8. CBC, Serum creatinine, USG of whole abdomen, X-ray chest in Dec 2011: As attached
9. USG of right breast 2D and core biopsy of right breast lump in Dec 2011: As attached

In the light of my mother's present condition, I feel that I should arrange for her treatment without further delay. I have gone through your website and I wish to bring my mother to your hospital for treatment. I was hoping that you would kindly consult the doctors at your hospital in perusing the attached reports and help me come to a decision about the next steps I should take. Kindly inform me whether the necessary diagnostic and treatment facilities will be available at your hospital, and any preliminary information about the stages and period of treatment, follow-up, visitors accommodation and an estimated total expenses for diagnosis and treatment, along with the methods of patient registration. I believe my mother is in a condition that requires rapid and competent hospital care, so you can perhaps understand my concern for immediate advice.

I wait for your kind and swift reply.

Yours Sincerely
Riton
