Zaki Tajwar Chowdhury
Date of Birth : Sept.26, 1989 Age : 21

FRUSTTRATED, NERVOUS, LACK OF INTELLIGENCE AND ENTHUSIASM

History :
Born only after 7 ½ month in mother’s womb.
Age : 15 days : serious comvulsion.
Age : 3 month : serious comvulsion
Age : 6 month : serious comvulsion

Was under treatment of Child Specialist M.R Khan at Nibedita Hospital; medicine given was Eunoctin and Diphaden.

Eunoctin and Diphaden was prescribed for taking regularly up to the age of 12, we later discontinued it at the age of 6. Recurrence of comvulsion used to take place at interval of about 4 month and the interval was becoming bigger and bigger as he was growing older. The last was probably at age of 12.

AT HIS EARLY AGE ( UPTO 12 ) WHILE HE HAD FEVER, IN MOST CASES IT WAS ABOUT 106.

HIS LOOK WAS BLANK EVEN AT THE AGE OF 6 MONTH AND COULD NOT MAKE HIS NECK STRAIGHT .

Child Hood was okay and he was an average student.
As he was growing gradually he started doing bad results.
From the age of 15 he gradually developed the habit of not talking with people or relatives around.
Now a days, he does not talk with anybody as long as he is asked anything. For example, even if some one of his age comes to our house he never says hello. If they ask something he rarely replies & very briefly. Does not want to drink enough water.

He has passed O Level Exams. Bengali, Mathematics, Economics and Accounting NOT ENGLISH.

He appeared in Accounting ( A Level ) failed , appeared again and waiting for result, we do not think he will come out successful.

He reads for a lot of time but not effectively. Say, he memorized something and writing it. The sentences in many cases will not be correctly written to mean something. MEMORY HIS VERY POOR – IT HAS BEEN COMPLAINED BY ALL HIS TEACHERS AND WE TOO KNOW IT FOR SURE. HE PROBALY, DOES NOT HAVE SOUND SLEEP.

WE ARE VERY MUCH CONCERNED THAT, HE WILL NOT BE ABLE TO MANAGE THINGS FOR HIMSELF IN ABSENCE OF HIS PARENTS.

we may come to India if you can assure us of good treatment for my son and advise the days & money required for treatment
